Flint Registry 2025 Survey
The Flint Registry’s follow up survey started rolling out in earlier in January. Survey invitations are going out in weekly batches. If you haven’t received yours, it may still be on the way. This survey will help continue to build the story about the long-term effects of the water crisis. Referrals to services will be made based on survey answers.
In appreciation for the time spent to finish the survey, all participants will receive a $50 electronic gift card via e-mail about 4-6 weeks after completing the survey. The survey will take about 45 minutes to complete. All the information you provide is strictly confidential.
Please go to www.flintregistry.org and contact us if you have any questions.
Why the 2025 Survey is Important
Your long-term involvement makes a difference. Your participation in the Flint Registry has helped shape state and national policy and advocated for local resources. This new survey will continue to ask about your health and wellbeing. With your permission we will use your answers to connect you to health-related services and programs.
The Flint Registry has improved the health of the Flint community with over 22,000 members and over 35,000 referrals to services. We remain committed to staying connected with Flint Registry members and continuing to advocate for resources to meet families’ needs. It is important that we hear from all members of the Flint community, those who were deeply impacted by the water crisis and those who were less impacted.
Did You or Your Child Recently Turn 18?
When you turn 18 years old, you are considered an adult and must complete health surveys for yourself with your contact information. As a child, a parent or guardian enrolled you in the Flint Registry and completed surveys on your behalf.
If you have turned 18 since originally enrolling, we kindly ask that you update your contact information to be able to complete the 2025 follow-up and future surveys. It is necessary for new adults to update their contact information and confirm their desire to continue to participate as an adult member of the Flint Registry.
Update contact information by visiting the following link below.
If you have any questions or need support completing the survey, please reach out to us at 833-GO-FLINT (833-463-5468).
